Patient Care Assistant - DME
Full-time, Days, 40 hours/week, Non-exempt
Position Summary: Coordinates ordering/delivery/pick-up of DME for home
patients working with Clinical staff, Vendors, Accounting, and Director of
Clinical Services. Prepares and disperses weekly Profile Packets and IDT
meeting documents. Maintain current and accurate medical records system.
Add incoming information to charts. Ensures that the essential documents are
included in each chart. Communicates with members of the health care team,
patients, and visitors using oral and written means and equipment such as the
telephone, intercom, fax, and pager. Maintains an orderly and businesslike
environment in order to assist members of the health care team in the
performance of their jobs despite high-stress factors. Works as part of Medical
Records team to meet Departmental goals. Other duties as assigned.
Position Qualifications: High school diploma or equivalent required. One year of related office experience. Proficient in clerical abilities related to
typing, filing and use of computer software and basic office machines. Must
have excellent interpersonal skills, phone communication skills. Knowledge
of medical terminology and durable medical equipment is preferred.
Physical Demands/Work Environment: While performing the duties of
this job, the employee is occasionally required to stand; walk; sit; use hands
to finger, handle, or feel objects, tools or controls; reach with hands and arms;
climb stairs; balance; stoop, kneel, crouch or crawl; talk or hear. Specific
vision abilities required by the job include close vision, distance vision, and the
ability to adjust focus. Have hearing acuity sufficient to hear verbal instructions
and communication from clients, respond to emergency alarms, and answer
the telephone. Ability to concentrate on moderate details with frequent
interruptions, attend to specific functions for 10-25 minutes, remember
multiple tasks and assignments over an 8-12 hour period, and understand
and relate specific ideas and concepts. Deal effectively with stress, patients
and their families, multiple tasks, noise, interruptions, and emotional loss, and
to work cooperatively with other members of the interdisciplinary team while
maintaining a pleasant and professional demeanor.

RN - Case Manager, Summersville
Full-time, 40 hours/week, Exempt, Days/On-call
Position Summary: Provides skilled nursing care, case management,
and other clinical support services to patient/families in the home setting.
Performs other related duties as required.
Position Qualifications: Current unrestricted WV license to practice
as a registered nurse. Two years of professional nursing experience in
hospice/home health preferred. Current driver’s license, reliable
transportation, and valid automobile insurance.
Physical Demands/Work Environment: Visual & hearing acuity sufficient
to auscultate BP, hear verbal instructions and communication from clients,
respond to emergency alarms, and answer the telephone. Must be able to
deal effectively with stress, patients and their families, multiple tasks, noise,
interruptions, and emotional loss, and to work cooperatively with other
members of the interdisciplinary team while maintaining a pleasant and

between the Hubbard Hospice House and the Hubbard Hospice House West,
located in Thomas Memorial Hospital.
Position Qualifications: One year experience as a CNA, nurse aide,
nursing assistant, or home health aide preferred.
