RN - Case Manager, Summersville
Full-time, 40 hours/week, Exempt, Days/On-call
Position Summary: Provides skilled nursing care, case management,
and other clinical support services to patient/families in the home setting.
Performs other related duties as required.
Position Qualifications: Current unrestricted WV license to practice
as a registered nurse. Two years of professional nursing experience in
hospice/home health preferred. Current driver’s license, reliable
transportation, and valid automobile insurance.
Physical Demands/Work Environment: Visual & hearing acuity sufficient
to auscultate BP, hear verbal instructions and communication from clients,
respond to emergency alarms, and answer the telephone. Must be able to
deal effectively with stress, patients and their families, multiple tasks, noise,
interruptions, and emotional loss, and to work cooperatively with other
members of the interdisciplinary team while maintaining a pleasant and
professional demeanor.

Patient Care Coordinator
Full-time, 40 hours/wk, Non-exempt
Days/Other shifts PRN
Position Summary: Under the supervision of the Clinical Services Director,
performs all duties related to providing telephonic, administrative and word
processing support for the patient care team. Communicates with insurance
and pharmacy companies. Assists with special projects and assists other
departments as needed. Performs other related duties as required.
Position Qualifications: High school diploma or equivalent required
with four years of office experience, (Prefer at least two years in a
medical/hospital/hospice office related position.) Proficient in clerical abilities
related to typing, filing and use of computer software and basic office
machines. Must have excellent telephone skills; strong verbal, written
and interpersonal skills and ability to interact with public.
Physical Demands/Work Environment: Work is normally performed in a
typical interior/office work environment. Ability to concentrate on moderate
details with frequent interruptions, attend to specific functions for 10-25
minutes, remember multiple tasks and assignments over an 8-12 hour
period, and understand and relate specific ideas and concepts. Deal
effectively with stress, patients and their families, multiple tasks, noise,
interruptions, and emotional loss, and to work cooperatively with other
members of the interdisciplinary team while maintaining a pleasant and
professional demeanor.
