If you have chronic heart failure
or lung problems,
you may be
eligible for this program if . . .
• You have breathlessness while getting dressed, walking short distances, talking or not doing much activity
• You are disabled from activity because of your shortness of breath
• You have had frequent ER visits or hospitalizations this past year
• You are on maximum doses of Diuretics and ACE inhibitors
• You have severe COPD or Stage IV COPD or FEV1 <30% predicted
• You have Stage IV Heart Failure or EF <20%.
 Did you know?
• The American Heart Association and American College of
Cardiology recommends hospice for severe heart failure.
• The Global Obstructive Lung Disease standards indicate that a FEV1<30% predicted is Stage IV lung disease or
Very Severe COPD.
• Hospice tends to the patient’s physical, emotional, and spiritual needs AND provides much-needed support for the family.
• A recent study shows that hospice patients live longer than those without this specialized care.
HospiceCare patients may choose to leave the program at any time during the illness. HospiceCare partners with the patient’s primary care physician— our patients do not have to give up their primary doctor.
