Interdisciplinary Team

Physician
Either your doctor or HospiceCare's Medical Director oversees your medical care.

Nurse
A registered nurse makes regular visists and serves as a case manager for each hospice patient. On-call nursing services are provided 24 hours a day/7 days a week for emergencies.

Social Worker
A licensed social workeraddresses the psychosocial needs of the terminally ill resident and his/her family. Social workers will coordinate the use of outside agencies as needed.

Certified Nursing Assistant
A CNA makes regular visits as needed to assist the patient and family with personal care.

Volunteer
Volunteers offer a helping hand and a listening ear for patients and their families. For more information, please contact volunteers@hospicecarewv.org

Chaplain
The chaplain is available to assist with spiritual concerns and needs on an individual basis.

Grief and Loss Counselor
Bereavement care is provided to family members and friends for at least one year after the hospice patient's death. Support groups are also available and open to the community.

Team members meet weekly to discuss how to provide the highest quality of care possible.
