A referral can be made by the patient, family, a friend, nursing facility or
doctor. Once the physician confirms the need for hospice services, a nurse and a social worker will visit to explain the plan of
care and to assess the patient’s needs, as well as those of the family. Services usually begin immediately after the patient is
admitted to the program.
Hospice is most effective when services are received early in the illness. Families repeatedly share that they wish they had called HospiceCare sooner so that their loved ones could have benefited more from its support and services.
Guidelines for HospiceCare admission:
- Individual has been diagnosed with a terminal illness and given a prognosis of six months or less, should the disease
follow its natural course.
- Curative measures are no longer effective or the individual wants to discontinue treatment.
- Individual resides in Boone, Braxton, Clay, Fayette, Greenbrier, Jackson, Kanawha, Lincoln, Mason, Monroe, Nicholas,
Pocahontas, Putnam, Roane, Summers or Webster counties.
- Individual requires comfort/palliative care.
Who Pays?
Medicare, Medicaid and most private insurance companies cover hospice services. For those patients without insurance, the agency
receives funds through community donations that help offset the cost of care.

It is the philosophy of HospiceCare that no person meeting the admission criteria will
be denied services because of sex, race, religion or inability to pay.
