Position Summary:
Provides nursing care and other support services to patients/ families in home/LTC facility, based on interdisciplinary plan of care under the direction of a Registered Nurse. Serves as LPN Crisis Care as needed. Performs other job duties as assigned.
Position Qualifications:
- Graduate of an approved school of practical nursing.
- Current unrestricted WV LPN license.
- Strong organizational, problem-solving, and people skills.
- Contingent upon successful completion of drug screen, background check, and other screenings as applicable.
Physical Requirements:
- Ability to lift, transfer, and reposition patients up to 50 pounds independently and over 50 pounds with assistance.
- Frequent standing and walking for extended periods (8–12 hour per shift).
- Frequent bending, stooping, kneeling, crouching, and reaching.
- Ability to push and pull equipment (e.g., wheelchairs, stretchers, and medication carts).
- Visual acuity to read medication labels, physician orders, and monitors.
- Auditory ability to assess patient needs and respond to alarms or calls for assistance.
Work Environment:
- Exposure to infectious diseases, bloodborne pathogens, and bodily fluids.
- Frequent interaction with patients, families, and interdisciplinary team members.
- Fast-paced clinical settings with potential for high stress.
- Required use of personal protective equipment (PPE).
